Required Forms

This is a comprehensive list of forms that parents may need to complete. Please ensure your forms are up to date and on file with the troop.

  1. Parents of kindergarten and first-grade Trailmen must complete and submit the Overnight Considerations for Fox Trailmen form to join the troop on overnight adventures.

  2. Every participant must have an up-to-date Youth Health Form or Adult Health Form on file with the troop. They are valid for one year.

  3. Any activity that involves being in more than knee-deep water requires that Trailmen pass a swim certification. The certification must be performed by a lifeguard or swim coach.

  4. Sixth through twelfth-graders and any adults that participate in more risky experiences must have a valid High Adventure Medical Form on file. This form must be completed by the participant's physician.

  5. Ask about becoming a Registered Adult. Registered Adults don't fill a leadership role, but they ensure safety by fulfilling our two-deep leadership requirement. We can tailor events toward boys' interests and abilities if we have enough Registered Adults to divide Trailmen into smaller groups.

  6. Alabama DHR Child Safety Training. Your son's safety is our top priority. That's why each adult participant who joins the troop on overnight adventures must undergo a criminal background check and state-certified child safety training. Please click here, log in, create your account and take this online test. There is no cost to complete the training and test. It takes about an hour to complete and can be taken over multiple sessions. After completion, please bring your certificate of completion to the next troop event.
